Care Coordination Network

This network was designed to enhance coordination of care and communication between the member hospitals and its member nursing facilities. The Merriman has been a member of the network since 2004. One of the metrics tracked by the CCN is 30 day hospital re-admission rates. The Merriman consistently has a low re-admission rate reflecting positively on the quality of medical and nursing care provided.

This network was established to maintain continuity of care across all points of care within Akron General Health System. The Merriman was among the first nursing homes to be granted membership in this network.
